Bears 2 large, fully preserved initials, I and F, and a smaller but elaborate, fully preserved Q, with 4 smaller initials, in green, red, and blue ink. Music on both sides. The bull (Luke) sitting on the large I is somewhat rubbed; blank portion of outer margin repaired. Written in a Beneventan script of the Bari type, in dark brown ink in 2 columns, 29 lines per full column.
