Date from colophon (photograph in Goshen-Gottstein, 149), restoring TLT Mʼʼ where a different word has been written over a rasura. The Patriarch "Athanasius" named there will be Athanasius Ṣalḥaya (986-1004) and the bishop "Isaac" will be Isaac of Callisura ordained by him.

Binding of full leather over wooden boards, with a flap. Velvet is pasted over the spine. The front and back boards are covered with plates of silvered brass, with repoussé images of Christ on the cross (back) and the Virgin and child with angels (front). The front image has an inscribed caption in Syriac script (but perhaps in Turkish).
